A lead form
A good form sends the details somewhere the owner opens: email, a sheet, or an inbox they check. A form saved only on the visitor’s computer reaches nobody. That is not a lead. It is an illusion.
Fields, a destination, and a test
Name and phone are the base. Email is optional if you do not need it to reply. A short message helps you understand the enquiry. A required field on everything discourages people.
Before launch, send a real enquiry and check that it arrived. After every change to the form, check again. A lead that never arrives is worth less than a pretty page.
